FYI this is not a "safe" solution but, I kept having this issue too and since I have quick release on my harness I decided to lock off my safety on my leash. I found that kiting in shallow water (3ft or less) and crashing I would inadvertently move the lease quick release out of position setting up ...

never tried launching unhooked, and as most of my launching is done unassisted, kite slide way, the pull on launch is quite big, so not sure how that would feel unhooked, in windy conditions I guess it would snatch the bar away, in 11 years I have never seen anyone at all try this, and in my iko/bk...

I've had this happen a couple of times. Seems to me if you just launched the kite with one hand on the chicken loop (like when you hook back in from an unhooked trick) and one hand on the bar it would eliminate this from happening at all.
