Does boat service mean that they follow you all the day or that they just drop you off in the morning on the island and pick up in the evening?

They charge per bag. But you have other option. Check lufthansa.com. - 12 hour flight in total. New-York - Munich - Bilbao. It is around 400 USd on sale now. You could check Livingston - www.laudaweb.it (charter flight from Rome and Milano,
no extra charge for luggage).
Airitaly - www.airitaly.com. Lowcost, you should pay per kilo for luggage.
As for me it was more convinient to fly with Lufthansa to Sao Paolo.
