Two ways to avoid it (well, shit can always happen!):
1. really tight bindings (boots) to not get out (recommended for wakestyle with harder landings)
2. really wide straps to get out quickly and have it rotating around the foot (recommended for anything else)
2 is my setup. had several crashes, one foot out and the other foot twisting inside the strap. This way the knee does not get twisted or anything else.
And yes, I always have grip in it.
